The general formula for constructing a 670 field is the following
(see DCM Z1, USMARC format for authority data, section 670, p. 1-
4):

     Main entry heading (optional). Title proper, imprint date or
     holdings:|b first location of data cited (data) other
     location (data)

Note: (1) The given names of the main entry (if a personal name)
are abbreviated (and without any dates or qualifiers), and the
main entry is now optional provided the title is distinctive;
     (2) the main entry can be a pronoun ("Hers", "His",
"Author's", "Its" [latter for corporate/conference headings]) if
the heading established is the same as the main entry;
     (3) the title is the title proper only and can be
abbreviated with marks of omission as well as abbreviated words
[e.g. "dict." for "dictionary"; same for location information but
not the data cited itself]; and
     (4) do not include brackets for either the imprint date or
location information (e.g. unnumbered page nos.) even if brackets
exist on the bib record that is the counterpart of the 670. See
the NACO participants' manual for fuller instructions as needed;
and
     (5) there is no ending punctuation.

NAME MAIN ENTRIES:

{The main entry for the 670 title above is Ordorica and a "His"
could proceed the title if desired [his name is also the 1xx in
the authority record] but the title is distinctive enough to not
require the main entry heading.}

Harbor Drive Linear Park, 1987:|bt.p. (Winn & Cutri)

{The bib record for the example above has title main entry.}

CITING OTHER DATABASES:

LC in OCLC, 1-9-91|b(hdg.: Crowshoe, Marilyn, 1953- )

{Notice that there is no colon preceding the subfield "b".}

OCLC, 3-3-95|b(hdg.: Kingston, John, fl. 1553-1584 ; usage: John
Kingston, J. Kingston, Ihon Kingston, Iohn Kingston, Jhon
Kingston, Kingston)

{Note this citation is for forms of the name found in non-LC bib
records and that subfield "d" coding has been removed from the
heading citation; also note that when citing databases you need
to group the forms of name found as headings (hdg.), i.e.,
1xx(s), 6xx(s), 7xx(s), etc. and/or as usage, i.e., in the body
of the transcription of the description (in the 245, 260, etc.).}

CITING REFERENCE SOURCES AND PHONE CALLS:

STC (2nd ed.)|b12723a (J. Kingston)

{You may use abbreviations of reference sources as commonly
applied or as found in citation form handbooks such as the
Standard citation forms for published bibliographies and catalogs
sed in rare book cataloging; the NACO participants' manual and
the USMARC format for authority data have some other abbreviated
forms in their examples.}

Phone call to the Water Utilities Dept., 12/7/90|b(the Utilities
Department changed its name to Water Utilities Department about
1970-71)

{See also p. 60 of the NACO participants' manual for other
examples from direct communications with sources.}
